We’re excited to announce that the South Umpqua School District Athletic Department has partnered with FinalForms, an online forms and data management system.
FinalForms allows parents and guardians to complete and electronically sign all athletic participation forms for their students. The best part-FinalForms saves your information from season to season and year to year, so you’ll never need t6 enter the same information twice!
You can review and update your information at any time, and you’ll only need to sign your forms once per year (or whenever updates are made).
Please note: FinalForms replaces all previous paper forms. All athletic paperwork will now be completed online through FinalForms.
We’re asking all parents/guardians of students planning to play sports at Coffenberry to create a FinalForms account and register your student(s).

Follow the prompts to create your account, add your student(s), and complete the required forms. When you create your account, FinalForms will send a confirmation email-be sure to check your inbox and follow the link to finish registration.
If your student’s physical form is already on file, the school will upload it for you. Once you’re registered, you’ll simply confirm which sport(s) your student will be playing.
You can also use FinalForms to list approved drivers who are allowed to take your student home from away games.
